Diabetes community team

Diabetes community team in Suffolk

We work with local GP surgeries in the Ipswich and East Suffolk area.  We are based at Ipswich Hospital and cross over from hospital care to GP surgery care to run clinics and share our specialist knowledge of diabetes.

If you need extra support managing your diabetes, your GP can refer you to us. We will offer you advice and guidance or an appointment to review your diabetes. Together with your surgery’s practice nurse, we will provide you with a plan to manage your diabetes.

We offer advice, support, and medication reviews if needed, to help you manage your diabetes.

Your GP will carry on performing most of your diabetes checks and annual screening.

Our aim is to empower you to manage your health more effectively to reduce the likelihood of further complications.

We work with the Wellbeing Suffolk (Opens in a new window) service, community dietetic services and medical teams.

Our diabetes educator leads our education groups and participates in the DESMOND programme (Opens in a new window) groups.

Diabetes education group for patients

The DESMOND programme  is a one day course to support people with a new diagnosis of Type 2 diabetes, designed to help you self-manage your diabetes. Up to 10 people can participate in a group.  You may bring a partner or friend with you on the day.

Diabetes education for nurses

We provide quarterly education sessions at the Ipswich Hospital diabetes centre, to share the latest information about diabetes.  We have sessions for Practice Nurses from GP surgeries and for Community Nurses.
