East of England Ambulance Service and control rooms
Community Integrated Neighbourhood Teams
Adult social care
Care homes for patients who are 18 or over and registered with an Ipswich and East Suffolk CCG GP.

Calls for REACT are made via the Care Coordination Centre

REACT service

REACT provide support for up to five days.

If long-term support is required REACT will refer on to the relevant services. REACT will ensure that these are in place prior to the withdrawal of REACT staff.
