REACT referrals and service

Referring to REACT

REACT accept referrals from:

• Patients and relatives
• NHS 111
• GPs
• East of England Ambulance Service and control rooms
• Community Integrated Neighbourhood teams
• Adult social care
• Care homes for patients age 18 and over registered with a GP in Ipswich or east Suffolk

REACT service

REACT provide support for up to five days.

If long-term support is required REACT will refer on to the relevant services. REACT will ensure that these are in place prior to the withdrawal of REACT staff.
